Schedule: Temporary 40 hours per week Monday - Friday 8:00 am - 4:30 pm

What Youll Do In This Role:
The Social Work Assistant is responsible for supporting Huron Valley PACEs Social Work Department to ensure seamless service and care coordination for participants and family members. This includes providing a variety of direct service to participants to ensure the continuity of care and collaboration of services. Works under the direct and indirect supervision of Huron Valley PACEs Social Work Manager.

The Social Work Assistant is responsible for supporting Huron Valley PACEs Social Work Department to ensure seamless service and care coordination for participants and family members. This includes providing a variety of direct service to participants to ensure the continuity of care and collaboration of services. Works under the direct and indirect supervision of Huron Valley PACEs Social Work Manager.
Key Responsibilities
- Coordinates care by receiving referrals collaborating with MSW team members the IDT and Medicaid specialists and connecting participants to appropriate resources.
- Manages Medicaid and Food Assistance processes including applications redeterminations verifications and ongoing status monitoring.
- Ensures compliance by reporting case changes to MDHHS tracking placements and length of stays and handling disenrollment notifications.
- Supports participants and families with forms advance directives guardianship processes and securing necessary documentation.
- Maintains accurate timely EMR documentation and communicates participant status updates while assisting with transitions between care settings.

Skills Knowledge & Expertise
- High School Diploma or GED required
- Bachelors degree in a human service-related field
- Experience working with the elderly
- Must have reliable transportation and a valid drivers license
- One year experience providing resource coordination
