Job Summary

Heres What You Need:

High School Diploma or equivalent (REQUIRED)

Four years of work experience in an Outpatient Healthcare Facility (REQUIRED)

Healthcare Training Certification (PREFERRED)

Customer Service work experience in Insurance or healthcare (PREFERRED)

CMA (STRONGLY PREFERRED)

Community Health Worker Certification (PREFERRED)

What You Will Do:

Educates and guides patients to understand the benefits and requirements of care management services as needed

Receives Chronic Care Management and Remote Physiological Monitoring orders from Neurology Physicians accesses patient charts and imports information into Care Management dashboard systems (Healtharc Prevounce Allscripts)

Engage patients by phone text or video to: Remind patients to record their vitals; respond to alerts related to patient symptoms or physiologic data; respond to patients for minor technical troubleshooting

Works with patients on determining insurance eligibility and authorization requirements for care management support

Assists patients with coordination of appointments and other care management needs related to their chronic conditions
