Position Summary

We are seeking a compassionate and highly organized Oncology Nurse Navigator to guide patients through the cancer care continuum. This role serves as a key point of contact for patients families and the multidisciplinary care team ensuring timely access to care coordination of services and a seamless patient experience.

The Oncology Nurse Navigator provides clinical expertise education and support from diagnosis through treatment and survivorship while helping to remove barriers to care.

Key Responsibilities

• Coordinate and ensure seamless evidence-based care across the oncology continuum
• Conduct comprehensive assessments and collaborate with the multidisciplinary team to develop individualized care plans
• Provide patient and family education support and advocacy addressing clinical and psychosocial needs
• Identify and address barriers to care to promote timely access and adherence to treatment
• Develop and deliver education materials and outreach programs for patients staff and the community
• Participate in quality improvement initiatives and maintain clinical expertise to support optimal patient outcomes
