Job Summary: We are seeking a skilled LMSW / Case Management professional to support discharge planning and care coordination within an acute care hospital setting. The role involves working closely with interdisciplinary teams to ensure safe and effective patient transitions across care settings.
Key Responsibilities:
Conduct discharge planning and care coordination for patients
Collaborate with physicians nurses and interdisciplinary teams
Assess patient needs and develop appropriate post-acute care plans
Support transitions across multiple hospital campuses
Document case management activities in electronic medical records
Ensure continuity of care for high-acuity and complex patients
Requirements:
Minimum 1 year of case management or acute care social work experience
Experience in discharge planning and care coordination required
First-time travelers accepted
Must be willing to work every other weekend
Experience in Med-Surg ED or long length-of-stay (LLOS) populations preferred
Active LMSW license required (RN licensure not applicable for this role)
BLS certification preferred
Must be comfortable working across multiple hospital campuses
