Patient Care Coordinator (Operations & EHR Coordination)

Location: Remote (Philippines)
Schedule: US Hours (Full-Time) 40hrs/week Mo-Fr
Compensation: $1800$2000/month performance bonus/commission DOE

About the Role

We are seeking a Patient Care Coordinator to support daily operational coordination across intake scheduling documentation and clinical communication. This role ensures that patient records scheduling systems and EHR data remain accurate and up to date across all workflows. This is a high-accountability role requiring strong English communication.

Key Responsibilities

Patient Intake & Record Creation

• Process patient referrals and intake requests
• Verify insurance and create patient records in EMR systems
• Ensure all onboarding documentation is complete and accurate

Scheduling Coordination

• Coordinate clinical team schedules and patient visit timing
• Maintain real-time updates in scheduling systems
• Support adjustments based on clinical or patient needs

Documentation & Data Entry

• Prepare and manage referral forms intake documentation and insurance records
• Maintain accurate EHR updates (HCHB or similar systems)
• Ensure timely and accurate data entry across all systems

Communication & Coordination Hub

• Route communications between patients families providers and clinicians
• Support field clinicians with scheduling and documentation needs
• Maintain HIPAA-compliant handling of all patient information

Required Skills

• Strong English communication skills
• Experience in healthcare administration coordination or data entry
• Strong attention to detail and system accuracy
• Ability to manage multiple workflows simultaneously
• Familiarity with EHR systems preferred
