Yee Hong is one of the foremost providers of culturally appropriate senior care in the country delivering high-quality resident experience and care to Chinese and other Asian seniors since 1994. The organization operates 805 long-term care beds and 308 seniors apartment units with an additional 224 long-term care beds opening in a new home by June 2026. In addition Yee Hong offers four campuses of managed care a hospice residence and a wide range of community-based senior and caregiver support services. Through this comprehensive network the organization serves more than 15000 individuals annually across the Greater Toronto Area.
In consultation with the Assistant Director (Department Head) the Program Manager of the Community Services plans develops coordinates and oversees the delivery of effective community services that reflects the organizations vision and strategic direction complies with relevant government legislations and policies optimizes available resources and maintains alignment across all program sites.
Responsibilities:
Plans develops coordinates and ensures delivery of effective community services that reflects the organizations vision and strategic direction and complies with relevant government legislations and policies
Optimizes government and non-government resources for the community services
Coaches develops and evaluates Program Coordinators supporting service teams to ensure effective staff deployment professional development and operational continuity to maintain a strong efficient team with high morale.
Assists in the development of internal budget for community services and implements effective expenditure control system for the community services. Ensures efficient and responsible use of resources
Develops and implements an effective risk management and quality improvement program for the community services
Cultivates and maintains a close network with community service agencies and Ontario Health atHome teams to ensure integrated and optimal client support.
Ensures effective deployment and development of volunteers to provide optimal support for the community services
Develops and updates policies procedures for the community services that are relevant appropriate and reflect the organizations vision
Develops and maintains an accurate and effective client information system
Keeps up-to-date with developments and best practices in the field
Other duties as assigned
Qualifications:
Bachelor degree in a relevant health or social services discipline or equivalent. Regulated health professional an asset.
Minimum 3 years supervisory experience in community-based services health care long-term care or a related human services setting. Experience working in a unionized setting an asset.
Knowledge and experience in community services within geriatric care setting and experience in staff supervision essential.
Experience supervising health care and/or social service professionals including multidisciplinary teams.
Strong computer literacy including Microsoft Office applications with the ability to learn and effectively use client management scheduling and reporting systems (e.g. AlayaCare Workday).
Satisfactory Criminal Reference Check (with vulnerable sector screen) result.
Good oral and written communication skills in English and Chinese.
Ability to travel to the different Yee Hong sites with the Greater Toronto Area.
