Responsibilities

As part of the regions largest health system you will join a collaborative therapy team dedicated to delivering evidencebased patientcentered care in a sevendayaweek environment. You will work with patients whopresent withdiverse and medically complex needs supporting them through comprehensive assessment treatment planning and education. This roleoperateswithin an interdisciplinary setting that values communication safety and highquality care.
In this role you will collect and assess patient data using evidencebased methods develop individualized plans of care and implement treatments while coordinating with patients families and the care team. You will organize care and discharge planning collaborate with pediatric discharge planners or CCS case managers when needed evaluate patient response and adjust treatment to support safe effective outcomes. You will also integrate research and qualityimprovement findings into practicemaintainrequired competencies support and mentor staff uphold ethical standards and meet expectations for clinical performance and documentation.
Qualifications
- Bachelors Degree in Physical Therapy required
- PT - State of California Physical Therapist License required
- BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
