Job Summary
Program Delivery & Client Support
- Support clients in setting and achieving goals related to housing financial stability and legal supports
- Deliver case management using a Housing First approach
- Advocate on behalf of client needs and goals
- Build and maintain strong relationships with community agencies and service providers
- Participate in case conferencing to coordinate effective service delivery
- Maintain accurate and timely client documentation and records
- Support or facilitate life skills and support groups for youth in Hestia Homes
- Stay informed on trends affecting youth experiencing low income mental health challenges and addictions
- Apply knowledge of trauma-informed person-centered harm reduction and Housing First approaches
Program & Organizational Support
- Maintain program statistics and support monthly and annual reporting
- Contribute to program evaluation processes (biannual evaluations)
- Assist in the development of new initiatives to enhance services
- Support onboarding and training of staff practicum students and volunteers
- Complete additional duties as assigned by the Program Manager
- Accurately track work hours using UKG mobile app (no missed punch-ins/outs)
- Submit expense claims accurately and on time

Qualifications
- Diploma (2-year) in Human Services OR a Bachelors degree in a related field
- Suicide Intervention training
- First Aid certification
- Minimum 1 year of related experience
- Valid drivers license reliable vehicle and $2M liability insurance
