Position Summary
Reporting as part of the Sun Parlor Home Leadership Team the Resident Care & Policy Lead plays a central role in supporting safe coordinated and person-centred care across the Home.
This position provides hands-on leadership and coordination for resident admissions transfers and discharges leads resident wellness reviews and responds to resident and family concerns using sound judgment discretion and collaboration with interdisciplinary leaders and subject matter experts the role also leads the coordination review and implementation of policies and procedures in alignment with the Fixing Long-Term Care Homes Act (FLTCA) and best practices.
The Resident Care & Policy Lead also supports performance management processes workplace investigations and confidential human resources matters as required while helping drive quality outcomes and a positive resident and family experience.

Working closely with the Administrator Social Worker Director of Care and interdisciplinary team members the Resident Care & Policy Lead helps drive quality outcomes and a positive resident and family experience.

Key Responsibilities
Policy & Practice Leadership

  • Ensure all home policies and procedures remain current compliant and aligned with the FLTCA and best practices
  • Lead policy review cycles implementation activities audits and continuous quality improvement initiatives
  • Provide guidance to staff related to resident rights professional practice and communication standards
  • Identify and escalate risks gaps or non-compliance concerns as appropriate
Resident Admissions Discharges & Transfers
  • Coordinate resident admissions internal transfers hospital transfers and discharges in collaboration with Social Work and Nursing Leadership
  • Support documentation assessments and communication to ensure accurate and timely transitions
  • Act as a key point of contact for residents and families during transitions ensuring clear and compassionate communication
  • Liaise with hospitals placement coordinators and external partners to support safe continuity of care
Resident Wellness Reviews (Care Conferences)
  • Plan schedule and coordinate resident wellness reviews in compliance with FLTCA requirements
  • Collaborate with physicians clinical teams and leadership to support meaningful care discussions
  • Participate as a member of the interdisciplinary care team and contribute to individualized care planning
Resident & Family Relations
  • Respond to resident and family concerns in a timely respectful and solution-focused manner
  • Support early resolution of concerns and assist with formal complaint processes when required
  • Ensure documentation and follow-up align with legislative and organizational requirements
  • Promote a culture of openness transparency and trust
Collaboration & Leadership Support
  • Serve as a liaison between residents families clinical teams and leadership
  • Lead or participate in interdisciplinary meetings care conferences and quality initiatives
  • Support onboarding and education related to resident care processes and transitions
Quality & Compliance
  • Support compliance with FLTCA regulations and Ministry requirements
  • Assist with preparation for inspections audits and quality reviews
  • Maintain accurate documentation and reporting related to admissions concerns and follow-up
  • Identify analyze and escalate trends and risks related to resident care and policy compliance
Other Duties
  • Perform other related duties as assigned by management
Qualifications
Education
  • Post-secondary degree or diploma in a health-related discipline (e.g. Nursing Social Work Health Administration Gerontology or related field)
  • Regulated Health Professional designation (RN RPN Social Worker) considered an asset
  • Ongoing professional development in areas such as long-term care legislation quality improvement risk management resident safety or conflict resolution preferred
  • Knowledge of the Fixing Long-Term Care Homes Act (FLTCA) and associated regulations is an asset
Experience
  • Minimum 35 years experience in long-term care or a comparable regulated healthcare environment
  • Experience supporting resident admissions transfers and discharges
  • Demonstrated experience in policy development quality improvement audits or compliance
  • Strong background in resident and family relations including concern or complaint resolution
  • Experience collaborating with interdisciplinary teams leadership and physicians
  • Exposure to performance management investigations or sensitive matters considered an asset
Knowledge Skills & Abilities
  • Strong knowledge of long-term care legislation resident rights and person-centred care
  • Excellent interpersonal communication and conflict-resolution skills
  • Ability to navigate emotionally sensitive situations with professionalism and empathy
  • Strong organizational skills with the ability to manage competing priorities and deadlines
  • Demonstrated discretion judgment confidentiality and decision-making ability
  • Proficiency with electronic documentation systems and standard office software
Other Requirements
  • Valid Ontario Class G drivers licence and access to a personal vehicle
  • Ministry of Transportation Driver Abstract (Uncertified)
  • Police Information Check (PIC)
