Summary

Practice in a newly expanded CentraCare-Sauk Crossing state-of-the-art facility designed to support team-based care improve patient flow and enhance both patient and provider experience. This role offers the opportunity to independently manage acute and chronic conditions provide preventive care and collaborate closely with on-site lab imaging and specialty resources. With modern technology updated clinical spaces and a strong focus on access safety and efficiency Sauk Crossing provides an ideal environment to deliver high-quality primary care while being part of an innovative and growing team.

Schedule

• Full-time 1.0 FTE Onsite CentraCare- Sauk Crossing Family Medicine Sartell MN

• Days 5 8-hour shifts 8:00am 5:00pm

  • Monday-Friday

• No nights weekends holidays or call responsibilities

Pay and Benefits

• Salary range:$109241.60 - $124904 per year

  • Pay may be determined by factors such as education experience skills knowledge location and internal equity

  • Salary and salary range are based on a 1.0 FTE reduced FTE will result in a prorated offer rate

  • May be eligible for additional pay through bonuses quality incentives or production-based (RVU) compensation.

Qualifications

Required:

• Graduate of an accredited Nurse Practitioner or Physician Assistant program

• Eligibility for Minnesota licensure as a Nurse Practitioner or Physician Assistant

• Prior experience as NP or PA in Family Medicine Walk-In Clinic Emergency Medicine or Urgent Care
