Your part in our Statewide Specialised Team
Reporting to the Dementia Care Manager Statewide Specialised Service you will support BlueCares neighbourhoods ininproviding supportguidance and strategies to personalcarersnursing and allied health staff in the delivery of high quality in home anddementia care.
The Dementia Care Consultantis responsible for ensuring the community service delivers quality Dementia care based on assessed clients needs that enhances and promotes quality of life and is consistent with current best practice.

The Dementia CareConsultant will deliver directin-home or telehealth support to people living with Dementia and their families by providing:
Information and tools specific to individual circumstances.
Assistance to accessBlueCare support services and programs tomaintain health andwell-being.
Carer Support.
Techniques to cope with individual behavioural concerns.
Prescribe meaningful activitiesand equipment from a comfort interventions libraryto engage people living with Dementia.
Care planning and assessment as per review schedule and change inclients needs.
Linkages and referrals external stakeholders and Government departments in the delivery of specialised dementia care whererequired.

Youll be an advocate for person-centred care leading by example through exceptional communication and customer with high level listening and problem solving skills you will have the ability to develop an understanding of the clients needs and providing quality advise in ensuring our clients ability to self-manage and live more independently. Youll present with:
Degree level qualifications in aNursing HealthCommunityService or other relevant discipline or extensive relevant work experience.
Certifications or qualificationsin dementia care.
Current AHPRA registration.
Extensive specialistknowledge and experience in Dementia assessment behaviour management planning care planning implementation and evaluation and Dementia mentoring and supervision alliedhealth or human services withdemonstrated skills reflective ofsignificantfunctional experience where scope of role includes clinical or care staff support.
Expert knowledge of services toolstechniques and resources toassist clients and staff.
Proven leadership capability within a multidisciplinary team.
Demonstrated comprehensiveunderstanding and knowledge of client centred community-based care services delivery.
Ability to liaise with the clinical and care team and to provide co-ordinated care.
Demonstrated experience and knowledge in Community Funding streams and programs.
Comprehensive knowledge of all relevant legislative regulatory requirements professionalstandards and guidelines relevant to the Aged Care Community Services environment with Quality Systems Accreditation WHS and Continuous Quality Improvement.
Demonstrated effective interpersonal and written communication skills including intermediate computer skills (e.g. Microsoft Office Suite).
