The Nurse Practitioner will provide comprehensive patient care in the home working as the clinical team leader for our home health palliative care and hospice teams.

Responsibilities

Perform comprehensive patient assessments and collaborate with facility staff on preventive services and early changes in condition recognition.
Manage acute and chronic conditions including medication adjustments.
Order and interpret diagnostic tests.
Develop implement and modify treatment plans as needed.
Participate in quality assurance and improvement activities.
Provide clinical education and training for patients families and facility staff.
Adhere to state-specific NP scope of practice requirements.
Demonstrates knowledge of etiologies risk factors pathophysiology and presentation of acute and chronic conditions.

Qualifications

Masters degree in Nursing or RN license.
Advanced Practice Nursing license.
NP Board Certification from recognized certifying body.
Credentialed with insurance companies preferred.
Fingerprint clearance.
Unrestricted prescribing and dispensing privileges with DEA licensure required.
Driving is a requirement for this position. Current state drivers license and proof of insurance is required.
1 to 2 years NP experience preferred
Experience with adult geriatric populations and population health management preferred
Strong communication organizational and customer service skills
Knowledge of Skilled Nursing/Transition Rehabilitation Centers (preferred)
Ability to handle a high volume of patients with skill and confidence
Knowledge and use of an Electronic Health Record
Ability to work with confidential information including HIPAA protected information
Ability to be proactive and initiate processes with little to no supervision

Pay Range

USD $150.00 - USD $160.00 /Hr.
