Position Overview
We are seeking a dedicated and compassionate Case Manager to join our Assertive Community Treatment (ACT) Team. The ACT Team Case Manager plays a critical role in delivering intensive community-based mental health services to individuals with severe and persistent mental illness. This position involves working collaboratively within a multidisciplinary team to support clients in achieving stability independence and improved quality of life.
Key Responsibilities
- Provide direct community-based services to clients including home visits and outreach
- Develop implement and monitor individualized treatment plans in collaboration with clients and the ACT team
- Assist clients with daily living skills housing stability employment and social integration
- Coordinate care with internal team members and external providers including healthcare social services and family supports
- Monitor client progress and document services in accordance with agency and regulatory standards
- Provide case management using motivational interviewing to help clients to utilize their strengths and abilities to address their problem
- Address risk factors such as substance use poor use of leisure time and self-doubt by applying problem-solving services
- Promote client benefits and income support by escorting consumers to appointments for Medicaid food stamps SSI/D HRA child support etc.
- Provide crisis intervention and support as needed
- Participate in team clinical meetings case conferences and on-call rotation as required
- Other duties as assigned
Qualifications
- Bachelors degree in Social Work Psychology Counseling or a related field (Masters preferred)
- Minimum of 6 months experience working with individuals with serious mental illness
- Knowledge of community resources and recovery-oriented practices
- Strong communication organizational and problem-solving skills
- Ability to work independently and as part of a multidisciplinary team
Preferred Qualifications
- Experience with ACT or other intensive community-based programs
- Bilingual skills are a plus
- Familiarity with electronic health records (EHR) systems
