Manchester CGL- Eclypse is looking to recruit a YP resilience worker within our dynamic YP-family service.

Working as a Young Resilience Worker you will support our service users and address any issues or concerns they have about drugs & alcohol. You will carry a caseload offering support to children YP and YA who are experimenting or using substances.
You will be part of a Multi-Disciplinary Team working with young people and families and will be taking a lead on working with young people at risk of exploitation. Case management is integral to the role and youwill deliver interventions using evidence based practice. This includes carrying out triage assessments care planning 1:1 sessions group work and digital interventions aiming on providing education brief advice but structured treatment and psychosocial interventions for children young people and young adults.

Hours: 37.5 hours per week (full time)

* Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles the salary and payments will be pro rata based on contracted hours
Responsibilities
About the role:
- Engage complex needs young people who have substance use issues and provide them with advice support and structured interventions to support their health and wellbeing needs. This may include facilitating access to and supporting clinical interventions.
- Build therapeutic relationships and deliver a personalised package of care.
- Carry out comprehensive service user assessments and develop individual care plans that clearly identify how achieving each goal will enable progression in addressing substance use and related outcomes.
- Work closely with your services Safeguarding Lead/Manager and Team Leader to ensure all issues and concerns are escalated and dealt with quickly and effectively.
- Further develop CGLs links with local children young people and family services. This includes adult drug and alcohol services.
- Support and provide advocacy for young people by participating in all relevant multiagency meetings including Child Protection meetings Complex Case Review panels and Team Around the Child meetings.
- Adopting a proactive approach to engagement maximising service accessibility and community venues. This includes delivering interventions peripatetically from our Manchester based offices home community venues schools colleges and Universities.
