We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in a skilled care setting in Phoenix AZ. This is a full-time opportunity offering competitive pay and consistent scheduling.
Schedule:
- Night Shift: 6:30 PM 6:00 AM (40 hours per week)
- Day Shift: 6:00 AM 6:30 PM (30 hours per week)
- 12-hour shifts
Responsibilities:
- Administer medications and perform treatments as ordered by the physician and within scope of license
- Monitor and report effectiveness of medications and treatments
- Maintain adequate supplies and medications
- Communicate resident needs with interdisciplinary team including family and staff
- Complete admission assessments and assist with care planning (MDS)
- Maintain accurate clinical and administrative documentation
- Participate in shift reports and team collaboration
Qualifications:
- Active LPN license (State of Arizona required)
- Knowledge of geriatric/long-term care practices preferred
- Strong communication and interpersonal skills
- Ability to assess respond and make sound clinical decisions
- Ability to work effectively in a team environment
