We are looking for aCase Manager Mental Health Professional (MHP) - Employment Specialistwho exemplifies that mission and who wants to work for an organization that makes a difference.

What will you be doing

The Case Manager MHP is responsible for developing and implementing individual programs to maximize the clients independent community living skills related to employment. The Case Manager MHP will also be responsible for coordinating services to meet criteria of Milestone SEP and IPS (Individual Placement Services) by following the guidelines of IPS and the Employment First Program to assist clients in obtaining and maintaining employment that is consistent with their preferences. The Case Manager MHP will engage in job development/networking in the community approximately 65% of the work week. Complete daily documentation of mental health services and IPS activities provided in accordance with state and agency standards as applicable. The Case Manager MHP is expected to work with a team of various service providers within the agency to ensure a positive client-centered approach. A minimum of one Client Centered Consultation for each client is required by all treatment team staff to ensure collaboration across programs with regular updates to maintain this collaboration.

Essential Job Responsibilities

  • Work scheduled hours and be flexible to meet client and program needs as assigned by Employment First Managers or Director. Includes cross training and coverage in other programs.
  • Work in accordance with IPS guidelines and other vocational providers.
  • Assure compliance with all agency state and federal regulations while providing services and completing assignments. Review and follow updated policies and procedures.
  • Facilitate communications and coordination of services with other AID staff and professionals in the community.
  • Maintain professionalism and good boundaries when working with clients coworkers and outside agencies.
  • Meet or exceed minimum service provision / billing (MRO) requirements as specified. Amt $6000 in CX 360.
  • Complete all required case management documentation (assessments treatment plans consents outpatient community paperwork) on a timely basis.
  • Engage clients toward the goal of competitive employment in community job settings.
  • Meet with all assigned clients on a regular basis depending on program and client needs; turn in completed assignment sheets or billing logs on a weekly basis.
  • Assure quality services and client satisfaction.
  • Contribute to clients IMCANS and ICAPS with vocational goals/objectives and providing information about clients updated progress towards these goals.
  • Develop community employment opportunities for clients referred.
  • Acquire and maintain certifications in First Aid Adult CPR CESP and Non-Violent Crisis Intervention (CPI) as well as other mandated trainings
  • Provide effective services for clients individual needs and in line with client rights and the Individual Placement Services Model (IPS).
  • Attend monthly clinical supervisions per DHS requirement
  • Attend program team meetings regularly (i.e. behavioral health supported living).
  • Conducts a minimum of six (6) in person contacts each week with people who have hiring authority.
  • Spends 65% of time in the community providing services to job seekers.

    Qualifications

    What will you bring to the table

    Education:Bachelors degree in Human Services or related field required or 5 years of service in Human Services field under the supervision of a qualified QMHP with either a High School Diploma or GED will be considered as substitution to fulfill for this requirement.

    License/certifications:Must obtain CESP certification after 1 year of employment.

    Experience: At least 1 year of experience in behavioral health or experience working with individuals with disabilities preferred.
    Physical:

    • Navigation of stairs
    • Ability to move about in the work environment and to regularly visit multiple work sites within the entire service area. Driving a car is required.
    • Ability to be able to assist clients in training in Labor focused positions.

    Additional Requirements:

    • The use of personal automobile a valid drivers license and the minimum amount of liability insurance as defined by AIDs Personnel Policy. Will require use of employees personal automobile.
    • Must acquire and maintain certifications in First Aid CPR Non-violent crisis intervention training CEUs and other relevant trainings
    • Flexibility in work hours required.
    • Computer skills experience with data entry word processing and spreadsheets.
    • Must be capable of verbal and written communication.
    • Neat and professional appearance.
