As a Hospice Admissions RN You will:
Visit patients (in homes hospitals nursing facilities etc.) who are referred for hospice care
Explain hospice services discuss goals and care with the patient and families answer any questions and obtain necessary consents and legal documents
Obtain medical history current medications and symptoms
Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
Coordinate the start of services (nursing aides equipment medications.)
Collaborate with interdisciplinary team
Maintain accurate and timely documentation

Qualifications
Graduate of an accredited nursing school with current licensure to practice in state of operation
One year of hospice nursing experience 3 years preferred or similar setting such as Long-Term Care Intensive care or Emergency room will be considered in place of hospice nursing experience
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
