Job Summary

POSITION SUMMARY:

The Geriatrics Case Coordinator coordinates the case processing functions within Elders Living at Home Program. The Case Coordinators are responsible for providing intensive assessment and case management services to homeless and at-risk elders referred to the program with the goal of helping them transition to and then maintain safe affordable permanent housing.

JOB REQUIREMENTS

EDUCATION:

Bachelors degree in psychology social work geriatric management public health or related field (or equivalent combination of formal education and related experience may be substituted for degree).

EXPERIENCE:

Prior experience working with adolescents teens and adults in the community required. Knowledge of health wellness/sport and recreation a plus.

KNOWLEDGE AND SKILLS:

  • Excellent communication interpersonal organizational and time management skills.
  • Cultural sensitivity and comfort with a wide range of social racial and ethnic populations.
  • Additional language skills (beyond that of English) appropriate to the patient population served is very helpful. Individuals from diverse cultural backgrounds preferred.
  • Excellent interpersonal skills to instill confidence and to advocate for patients and their family.
  • Motivation and self-direction; ability to prioritize competing responsibilities in fast-paced clinical care setting.
  • Computer competency inclusive of the ability to enter and access data required.

Compensation Range:

$25.48- $37.02

This range offers an estimate based on the minimum job qualifications. However our approach to determining base pay is comprehensive and a broad range of factors is considered when making an offer.
