POSITION FUNCTION:
The Dari-speaking Activity Coach provides direct program delivery and one-on-one participant support for Choose to Move an initiative that supports older adults in increasing their physical activity and social connectedness. The coach facilitates group meetings conducts individual consultations and maintains linkages with community resources to enhance participant engagement.
KEY DUTIES AND RESPONSIBILITIES:
- Participate in training provided by the Active Aging Society to deliver the Choose to Move (CTM) program effectively.
- Conduct outreach and promote CTM to older adults in diverse communities.
- Organize and facilitate intake sessions and consultations to assess participate readiness using the Get Active Questionnaire.
- Support participants in creating individualized physical and social activity action plans.
- Facilitate eight group meetings per CTM cycle fostering peer support and healthy habit-building discussions.
- Maintain regular contact with participants (weekly in the first 6 weeks bi-weekly in the final 6 weeks).
- Create an inventory of community-based physical and social activity opportunities and connect participants to these resources.
- Promote safe participation by following MOSAICs policies and procedures
- Support the coordinator or manager to ensure completion of deliverables and contractual outcomes of the program or project
- Maintain timely and accurate documentation in program databases.
- Participate in evaluation activities to assess participant progress and program outcomes.
- Bridge participants to appropriate community services and programs.
- Build relationships with local service providers to expand the referral network.
- Represent MOSAIC and advocate for seniors wellness and inclusion in community settings.
- Work with the coordinator or manager as requested to involve regional/community stakeholders increasing service accessibility for the client group
- Publicize or promote MOSAIC programs/services to regional and community stakeholders
- Participate in departmental all-staff and relevant community and province-wide meetings in-service training and other training
- Regularly update the program outcomes and targets to the supervisors assist in the preparation and submission of reports (proposals budgets program/project- data) in a timely fashion and in a format prescribed by the agency/funder to meet funding requirements and contribute to continuous quality improvement
QUALIFICATIONS:
- Knowledge of programs and services service linkingtechniques project coordination and community development practice immigrant settlement issues and needs; standard office equipment; knowledge of standard MS 365 applications; client database management systems specialized knowledge related to the program/project
- Post secondary degree college diploma or training certificate in Social or community services recreation gerontology adult education or a combination of training and experience relevant to the program project or service
- At least two years experience working with older adults in community or health-related settings; facilitation training and public speaking providing assessment and referral services; working with those from diverse cultural backgrounds; lived personal experience is an asset; experience working with newcomers and vulnerable populations desirable; specialized experience related to the program/project
- English Communication skills other languages an asset
- Experience in facilitation & coaching (interactive workshops and sessions online or in-person as applicable)
- Engagement with program/projects client age group
- Able to work independently and with a team
- Cultural awareness
- Organized and able to set priorities and meet deadlines
- Tactful discrete diplomatic patient flexible
- MS 365 suite publishing software database(s) Zoom
- Ability to take initiative
- Able to set professional boundaries with clients
- Valid First Aid/CPR certification an asset
- Current satisfactory Criminal Record Check is required
- Access to reliable transportation may be required
COMPENSATION: $28.58 - $31.03/hour
HOURS: This is an on-call position starting from May 11 2026 to July 31 2026.
The hours can be approximately 6 hours per week a maximum total 72 hours over 12-week period.
Availability for flexible work hours including some evenings and weekends.
The successful candidate must be able to commute to Boundary office 5575 Boundary Rd Vancouver BC.
BENEFITS:
A competitive benefits package is provided with contracts of at least 1 year in length and which offer a minimum of 17.5 hours of work per week.
