JOBSUMMARY: M-F $18.00-$23.00

Theprimary purpose of the Social Worker position is to providesupport toresidents and their families in coping with placement in a long-term carefacility under the direction of the Social Services Director and to assureresident safety.

JOB DUTIES& RESPONSIBILITIES:

Performassessment of the resident at admission upon change of condition and/orannually; create review and update care plan and progress notes

Assistin the admission process admission contracts PAE and PASSAR.

Providedirect psychosocial intervention

Assistresidents family in coping with placement in a skilled nursing facilityphysical illness disabilities of the resident and the grieving process

Coordinateresident visits with outside services - dental optical etc.

Workwith the resident family and other team members to plan discharge.

Participatein-service programs to educate staff regarding psychosocial issues and residentrights performed by the Social Services Director

Attendand document resident counsel meetings

Conductjob responsibilities in accordance with the Companys Code of Business Conductthe Corporate Compliance Agreement appropriate professional standards andapplicable state/federal laws.

Musthave knowledge of psychosocial practices applicable to a long-term careenvironment.

Willwork cooperatively as a member of a team.

Needto communicate effectively with residents their families and at all levels ofthe organization and maintain confidentiality.

JOBREQUIREMENTS:

Education

Preferredbut not required - Bachelors degree from an accredited college or universityin a human services field such as social services sociology psychologyspecial education or rehabilitative counseling.

Experience

Musthave a minimum of one (1) year of experience in social work working directlywith residents in a health care setting.
