Are you passionate about helping individuals improve their mental health and overall well-being We are seeking an experienced Behavioral Health Specialist to join our collaborative outpatient care this role you will work alongside primary care providers care managers and other healthcare professionals to support patients through counseling treatment planning and ongoing behavioral health services.
This is an outpatient-only opportunity with a predictable schedule no hospital call and the ability to build meaningful long-term relationships with patients.
What Youll Do
Conduct comprehensive behavioral health and mental health assessments
Develop individualized treatment plans tailored to each patients needs
Provide outpatient counseling and therapy services to adults and/or children
Deliver crisis intervention and behavioral stabilization support when needed
Maintain a manageable caseload focused on quality and continuity of care
Collaborate with physicians care managers and interdisciplinary teams
Connect patients with community resources and behavioral health services
Complete accurate documentation treatment plans and progress notes
Monitor patient progress and adjust treatment approaches as appropriate
Prepare discharge summaries and transition plans
Requirements
What Were Looking For
Masters degree in Social Work Counseling or a related field
Active LPC or LMSW license
At least 3 years of outpatient behavioral health experience
Strong background in assessments treatment planning therapy and crisis intervention
Excellent communication organization and teamwork skills
Comfortable working in a structured integrated care environment
Dedicated to serving vulnerable populations and improving patient outcomes
