Behavioral Health Specialist Outpatient Integrated Care
We are currently seeking a licensed Behavioral Health Specialist (LPC or LMSW) to provide high-quality outpatient behavioral health services within an integrated care setting. This role is designed for an experienced clinician who values collaboration structure and evidence-based treatment.
The Behavioral Health Specialist will work closely with primary care providers and interdisciplinary team members to support patients with behavioral and mental health needs in an outpatient environment.
Primary Responsibilities
- Perform psychosocial and mental health evaluations
- Develop implement and update individualized treatment plans
- Provide outpatient psychotherapy and counseling services
- Respond to behavioral health crises and provide stabilization interventions
- Manage an organized caseload while maintaining quality standards
- Coordinate care with physicians case managers and other clinical staff
- Identify and facilitate access to community-based resources
- Maintain timely accurate and compliant clinical documentation
- Track patient outcomes and modify treatment strategies when necessary
- Complete discharge planning and case summaries
Requirements
Required Qualifications
- Masters degree in Counseling Social Work Psychology or related field
- Current LPC or LMSW license in good standing
- Minimum 3 years of outpatient behavioral health experience
- Proven experience in assessment diagnosis treatment planning and crisis management
- Strong interpersonal communication and documentation skills
- Ability to thrive in a collaborative multidisciplinary setting
