Nursing Vault Plus Staffing is looking for a Speech-Language Pathologist (SLP) for our client at Lofland Park Center (a skilled nursing/rehabilitation center) . This is a 13-week contract assignment. The SLP evaluates diagnoses and treats residents with speech language cognitive and swallowing disorders. They develop individualized care plans to improve functional independence document progress and collaborate with physicians and families.
Key Responsibilities
- Evaluations & Treatment: Assess residents to identify communication cognitive or swallowing deficits and provide skilled therapy.
- Care Planning: Develop implement and update individualized treatment plans in accordance with physician orders.
- Dysphagia Management: Assess and manage swallowing disorders including diet modification recommendations.
- Documentation: Maintain accurate timely medical records of treatment and progress to ensure compliance with regulations.
- Collaboration: Participate in interdisciplinary team meetings including rehabilitation nursing and resident care conferences.
- Education: Provide training to family members caregivers and staff regarding resident care and communication strategies.
Requirements
- Education and Certification: Masters degree in Speech-Language Pathology and state license to practice.
- Certification: Certificate of Clinical Competence (CCC-SLP) is preferred.
- Geriatric & Clinical Expertise: Strong skills in assessing and treating swallowing disorders (dysphagia) speech disorders (aphasia dysarthria) and cognitive-communication impairment.
- Documentation and Compliance: Proficiency in Electronic Medical Records (EMR) and adhering to federal/state regulations.
- Interdisciplinary Collaboration: Ability to work effectively with doctors nurses physical therapists and occupational therapists.
- Soft Skills: High levels of compassion empathy and patience when working with elderly patients and their families.
- Adaptability: Flexibility to manage changing busy caseloads in a skilled nursing facility setting.
