Responsibilities
- Conduct comprehensive patient assessments and physical examinations
- Develop and implement individualized treatment plans
- Prescribe medications and monitor patient progress
- Collaborate with physicians and other healthcare professionals
- Educate patients and their families on health management and disease prevention
- Document patient care activities accurately and timely
- Participate in patient care coordination and follow-up
- Stay updated on medical best practices and internal medicine advancements
Requirements
- Active Nurse Practitioner or Physicians Assistant license
- Associate degree in a relevant field; Bachelors degree preferred
- Experience in internal medicine or a related specialty
- Strong clinical assessment and diagnostic skills
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a fast-paced environment
- Detail-oriented with strong organizational skills
- Commitment to patient-centered care and continuous improvement
Schedule: Monday-Friday 8a-5p
Patient Population: Ages 18 specializing in geriatric care
Duties: Routine physical exams and health screenings sick visits treatment and management of chronic conditions same-day appointments immunizations and medication prescriptions and refills
Must be board-certified
