You will work remotely with a collaborative interdisciplinary team to improve patient outcomes and reduce healthcare costs through innovative coordinated care.
This position is ideal for an NP with strong leadership skills who thrives in a dynamic startup environment and is deeply committed to caring for vulnerable populations. We are hiring for both a Nurse Practitioner and a Lead Nurse Practitioner position (with higher compensation for the Lead role).
Requirements
1 years of experience as a licensed Nurse Practitioner
Active/clear Nurse Practitioner license to practice in the State of Arizona
Active/Clear DEA
1 years of experience working in a rehab setting /rehabilitation facility centers
Strong knowledge in HIPAA compliance as it pertains to telehealth
Key Responsibilities
Serve as the operational lead for the interdisciplinary care team.
Lead care coordination efforts for complex Medicaid populations.
Practice autonomously with prescribing authority.
Manage care delivery primarily through telehealth and remote coordination.
Coordinate care across multiple providers including primary care physicians and health plan case managers.
Support proactive care management including risk stratification and early detection of health declines.
Provide 24/7 clinical access to help patients avoid unnecessary Emergency Room visits.
Support patients during hospitalizations and post-discharge transitions.
Help build and shape a new innovative value-based care model.
Schedule and Compensation
Schedule: Primarily a virtual care model with collaborative interdisciplinary teams.
Compensation: The base salary can range from $130000 to $175000 depending on experience and whether the role is a Leader position.
