Telemental Health Clinician II BH Service Line Virtual Transition Care Team PT (20 hours)

2 10-hour shifts per week ( Saturday/Sunday 0830-1900)

Job Summary

We are excited to announce an opportunity to join ourVirtual Transition Careteam as a Telemental Health Clinician this roleyou would be providing supportto patients who aredischarging from acute behavioral health team iscomprisedofmental health cliniciansthatprovidetelephonicoutreach toaddress individual needs connection tocommunityresources and emotional support topatientspost-discharge.

This position offersflexibilityof remote work while making a meaningful impact on patient outcomes.

Essential Functions

Provide telephonic outreach and support for post-discharge patients.

Conduct assessments and psychosocial evaluations using DSM criteria.

Implement crisis plans and provide interventions for high-risk patients (suicidal homicidal psychotic aggressive).

Administera varietyof clinically validated screening tools and assessments.

Maintain current knowledge andexpertisein behavioral health and treatment modalities.

Identifyand resolve barriers to accessing behavioral health services.

Adhere to professional standards and licensing board ethics.

Communicate effectively (written and verbal) with medical staff leadership and team members.

Document assessments and progress updates in the medical record per departmental standards.

Demonstrate sound clinical judgment and decision-making in a fast-paced environment.

Participate in policy development implementation and evaluation.

Collaborate with team members to managecase load.

Physical Requirements

Work is completedvirtually froma home office withlong periodsof sitting.

Ability to workshiftsup to12-hourshifts.

Must be able to travel to the virtual hub in South Charlotte (28209) if connectivity issues arise.

Mustresidewithin 150 miles of the virtual hub (1601 Abbey Place Charlotte NC 28209).

Education & Experience Requirements

Masters degree in Social Work Counseling Psychology or Marriage and Family Therapy from an accredited graduate school or Associates or bachelors degree in nursingbut requires full clinical licensure with 2 years experience required

Full current licensure inNorth Carolina: LCMHC LCSW LPA LMFT

Minimum2 years of clinical experiencerequired.

Experience in behavioral health emergency/crisis work and virtual/remote care working with suicide risk assessment & safety planning preferred
