Valley-Wide Health Systems is seeking a compassionate and collaborative Behavioral Health Provider to deliver integrated whole-person behavioral health care within our team-based primary care model supporting patients mental emotional and social well-being across the lifespan.
Key Responsibilities
- Serve as a Behavioral Health Provider (BHP) within a team-based care (TBC) model delivering whole-person (biopsychosocial) care.
- Identify and assess at-risk patients including crisis evaluation and intervention.
- Conduct diagnostic mental health assessments and screenings; determine level of care and refer to specialty services as needed.
- Develop and implement biopsychosocial treatment and relapse-prevention plans in collaboration with patients families and care teams.
- Provide evidence-based behavioral health interventions and psychotherapy.
- Monitor patient progress ensure timely and accurate documentation and communicate updates with the interdisciplinary care team.
- Collaborate with internal and external partners including Enabling Services to address social determinants of health.
- Participate in integrated care meetings and support the ongoing development of integrated behavioral health infrastructure.
- Maintain professional relationships with community agencies healthcare partners and local organizations.
Requirements
- Masters or Doctoral degree in psychology counseling or clinical social work.
- Licensed or license-eligible in Colorado as a Behavioral Health Provider (LCSW LPC LMFT PsyD or PhD).
- LPCCs must complete the Jurisprudence Exam and obtain LPC licensure within three years of hire.
- Two years of clinical experience preferred.
- Knowledge of mental health assessment psychotherapy crisis intervention and DSM-5/ICD-10.
- Ability to pass a behavioral health background check.
- Valid Colorado drivers license with ability to travel.
- BLS certification provided upon hire.
