About the role
Play a vital role within the Chronic and Complex Care (CCC) team providing a mix of home-based and clinic-based support to individuals living with chronic conditions.
Provide comprehensive social work support to patients families and carers affected by chronic health conditions including heart failure respiratory conditions and neurological disorders.
Contribute clinical expertise to crisis work patient flow complex and end-of-life care while providing consultation and education to staff and patients.
Apply advanced clinical knowledge and skills within the relevant area of practice and contribute to research quality improvement and departmental initiatives.
Actively participate in weekly multidisciplinary team (MDT) case reviews contributing to coordinated care planning and decision-making whilst identifying opportunities for service innovation and implementing evidence-based improvements.
About you
Current unrestricted NSW Drivers License.
Hold a degree in Social Work and be eligible for membership with the Australian Association of Social Workers (AASW) with demonstrated experience in clinical health social work settings with high level skills in aged care social work service.
Demonstrated competencies in relevant social work practice frameworks including: human rights social justice crisis intervention adjustment to illness and disability short term solution focused intervention grief and loss complex assessment advocacy care and discharge planning.
High level understanding and experience of the complex social issues for patients families and carers in regard to aged care services rehabilitation disability and residential care.
Demonstrated experience in multidisciplinary teamwork and clinical supervision with the ability to lead clinical planning research quality and education initiatives supported by strong communication time management and technical skills.
Sound knowledge of legislative requirements and the social work role in regard to domestic violence elder abuse child protection guardianship and victims of crime.
