What Youll Do

The Patient Referral Coordinator oversees and coordinates patient referrals to ensure timely accurate and seamless care and follow-up. This role works collaboratively with providers patients and external partners to facilitate the referral process maintain precise documentation and promote continuity of care across the patient journey.

• Coordinate and track patient referrals from initiation through completion ensuring timely and seamless care
• Verify patient demographic insurance and clinical information and obtain prior authorization as needed
• Communicate with patients to explain referral details next steps and assist with scheduling appointments and follow ups
• Maintain accurate documentation in the electronic medical record including referral status and related communications
• Collaborate with providers clinical teams and external partners to support effective care coordination and continuity of care
• Performs a variety of tasks and other duties as assigned

Were seeking a detail-oriented adaptable professional with strong communication skills who thrives in a fast-paced environment. The ideal candidate is comfortable with EMR systems works well with others and is committed to accuracy confidentiality and a patient-centered experience.

• High school diploma or equivalent preferred
• 2-3 years of experience in a clinic setting preferred

• Pay Range: $18.90 - $23.62 /hour (based on experience)
