Job Summary:
We are seeking a Licensed Master Social Worker (LMSW) to support case management and care coordination services in an acute care hospital setting. This role focuses on discharge planning patient advocacy and interdisciplinary collaboration across multiple hospital units.
Key Responsibilities:
Provide case management and discharge planning services for hospitalized patients
Coordinate care transitions and post-discharge services
Collaborate with physicians nurses and interdisciplinary teams
Assess patient needs and develop appropriate care plans
Support patients in navigating healthcare and community resources
Document all interventions and care coordination activities in EMR
Work across multiple hospital units as needed
Schedule & Work Details:
5x8-hour day shifts (7:30 AM 4:00 PM)
Every other weekend required
Floating required across hospital campuses
Guaranteed 72 hours per 2-week period
Requirements:
Active Washington LMSW license (required)
Minimum 1 year of experience in acute care social work
Experience in discharge planning and care coordination required
BLS certification required
Ability to work across multiple hospital units
Preferred Qualifications:
Experience in Med/Surg ED and length-of-stay case management
Strong interdisciplinary communication skills
Experience in large hospital systems
