Job Description
We are seeking a dedicated Nurse Practitioner (NP) to provide high-quality care in post-acute this role you will support patients during their recovery after hospital discharge helping to reduce readmissions and improve overall outcomes. This position offers a consistent weekday schedule autonomy and a collaborative team environment.

Schedule:
Monday Thursday (Daytime Hours)

Key Responsibilities:

Conduct rounds in assigned facilities on a consistent weekday daytime schedule.
Perform focused patient assessments to monitor recovery manage pain wounds and functional decline.
Collaborate with physicians therapists and facility staff to develop individualized care plans.
Complete documentation using an electronic health record (EHR) system with remote flexibility.
Identify and address readmission risks and gaps in post-acute care.
Ensure compliance with applicable clinical and regulatory standards.

Qualifications:

Masters degree in Nursing with certification as a Nurse Practitioner (NP) from an accredited program.
Active unrestricted NP license with prescriptive authority in the state of practice.
2 3 years of experience in PM&R geriatrics post-acute care or a related field preferred.
Experience working in nursing facilities is a plus.
Strong clinical skills in managing recovery rehabilitation pain and chronic conditions.
Excellent communication and time-management skills.
Proficiency with EHR systems and remote documentation.
Ability to work independently with a high level of autonomy.
