We are seeking a dedicated Nurse Practitioner (NP) to provide high-quality care in post-acute this role you will support patients during their recovery following hospital discharge helping to reduce readmissions and improve overall outcomes. This position offers a consistent weekday schedule autonomy and a collaborative team environment.

Key Responsibilities:
- Conduct rounds in assigned facilities on a consistent weekday daytime schedule.
- Perform focused patient assessments to monitor recovery manage pain wounds and functional decline.
- Collaborate with physicians therapists and facility staff to develop individualized care plans.
- Complete documentation using an electronic health record (EHR) system with remote flexibility.
- Identify and address readmission risks and gaps in post-acute care.
- Ensure compliance with applicable clinical and regulatory standards.

Qualifications:
- Masters degree in Nursing with certification as a Nurse Practitioner (NP) from an accredited program.
- Active unrestricted NP license with prescriptive authority in the state of practice.
- 2 3 years of experience in PM&R geriatrics post-acute care or a related field preferred.
- Experience working in nursing facilities is a plus.
- Strong clinical skills in managing recovery rehabilitation pain and chronic conditions.
- Excellent communication and time-management skills.
- Proficiency with EHR systems and remote documentation.
- Ability to work independently with a high level of autonomy.
