Youll step into a community-based role focused on supporting older adults and their families through both short- and long-term case management. Youll develop individualized care plans conduct home visits and maintain ongoing communication with your caseload to help clients remain independent. Youll have autonomy in managing your schedule while supported by a dedicated team.
