Care Coordinator & Nurse Mentor (Senior)

Infinite Aged Care (Knoxfield) is seeking an experienced Aged Care Clinical Care Coordinator to join their team.

The Role

Join the Knoxfield Clinical Care team in a rewarding and fast-paced role as a Care Coordinator and Nurse Mentor. This full time role reports to the General Manager with support and guidance from the Clinical Care Manager whilst Supervising and supporting our passionate care key purpose of this role is to ensure that all our residents within our beautiful facility in Knoxfield receive the highest quality of clinical care. The Clinical Care Coordinator is responsible for the management and resourcing of the care needs within the facility documentation and assessments are up to date and in alignment with best-in-class clinical practices.

Key Responsibilities

• Provide strategic and operational leadership to the clinical care team to ensure highquality residentcentred care.
• Oversee the development implementation and regular review of individualised care plans aligned with best practice.
• Monitor clinical activities including mealtimes and key daily events to maintain consistent service excellence.
• Mentor and support RNs ENs and PCWs to strengthen clinical capability teamwork and professional growth.
• Collaborate with multidisciplinary teamsincluding allied health GPs and hospitalsto coordinate holistic resident care.
• Facilitate effective communication processes such as shift handovers clinical huddles and team meetings.
• Provide oversight of clinical decisionmaking ensuring compliance with evidencebased practice and regulatory standards.
• Manage complex resident care needs including specialist referrals case reviews and family consultations.
• Lead onboarding training and ongoing development of care staff identifying skill gaps and supporting performance management.
• Monitor resident health changes escalate concerns promptly and implement strategies to minimise risks such as falls and infections.
• Participate in clinical audits maintain accurate documentation and support quality improvement initiatives aligned with Aged Care Quality Standards.
• Coordinate daily workflow staffing and resource allocation while maintaining communication with senior leadership on team performance and resident outcomes.

You will offer

• You will have an innate empathy for our residents and be committed to continually improving the quality of clinical care provided to them.
• Minimum 2 - 3 years experience as a Clinical Care Coordinator in an aged care setting
• Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA)
• Post graduate qualifications in aged care would be advantageous but not essential.
• Experience and extensive knowledge of Aged Care clinical assessments care plans AN-ACC and accreditation compliance and documentation.
• Engaging style that has ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment.
• High level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements.
• A confident team player with excellent communication and interpersonal skills that has a passion for leading clinical outcomes.
• Ability to manage multiple stakeholders and have a balanced and level-headed approach to emergency and stressful situations that arise.
• Have a valid NDIS Workers Screening Clearance (or ability to apply via How to apply NDIS).
