Responsibilities
Assist clients with Activities of Daily Living (ADLs) such as bathing dressing grooming toileting feeding and meal preparation.
Provide personal care and behavioral support for individuals with developmental and intellectual disabilities.
Support mobility transfers and safe ambulation as needed.
Monitor client health observe and report behavioral or medical changes to the RN or supervisor.
Follow individualized care plans while maintaining client rights and confidentiality (HIPAA compliant).
Engage clients in meaningful activities community outings and social interactions to promote mental and emotional well-being.
Maintain a clean safe and organized home environment.
Communicate and collaborate with families case managers social workers and healthcare professionals.
Document care and communication clearly and accurately.
Skills & Experience
First Aid & CPR certification (provided if you dont have it)
Experience in group homes supported living or assisted living highly desirable
Strong observation skills for monitoring behavioral or health changes
Experience working with individuals with autism Down syndrome or other developmental disabilities
Ability to lift transfer and assist with mobility (up to 50 lbs)
Cooking ability for simple and dietary-specific meals
Home care home health or hospice experience is a plus
Excellent communication and teamwork skills
Qualifications
High school diploma or equivalent
At least 6 months of experience supporting individuals with developmental disabilities
Negative TB test or clear chest X-ray
Pass pre-employment drug screening
Pass physical examination
LiveScan background check
Valid drivers license and reliable transportation preferred
