Lutheran Social Services of Wisconsin and Upper Michigan (LSS)

Status: FullâTime 40 hours/week

Schedule: Primarily daytime hours with some flexibility based on client needs

Location: Baraga Houghton and Keweenaw Counties MI

Program Hours Breakdown:

  • 30 hours/week Superior Futures for Youth

  • 10 hours/week Rapid ReâHousing (RRH) Program

Lutheran Social Services of Wisconsin and Upper Michigan (LSS) is seeking a dedicated Case Manager to support youth and young adults ages 1224 experiencing homelessness through our Superior Futures for Youth addition to youth services this role includes 10 hours per week supporting the Rapid ReâHousing (RRH) program a lightâtouch model that assists adults and families experiencing literal homelessness in securing permanent housing and maintaining stability through shortâterm rental assistance.

This is a fullâtime benefitâeligible position offering the opportunity to make a meaningful impact in the lives of vulnerable young people and households experiencing housing instability.

Essential Duties and Responsibilities

Superior Futures for Youth (SFFY) 30 Hours/Week

  • Conduct assessments develop case plans and provide ongoing case management for youth ages 1224.

  • Coordinate community social and mental health services to meet client needs.

  • Maintain accurate case records prepare required reports and participate in hearings as needed.

  • Build strong relationships with clients partners and community agencies.

  • Attend trainings staff meetings and community events; represent LSS professionally.

  • Support agency quality initiatives and perform additional duties as assigned.

Rapid ReâHousing (RRH) 10 Hours/Week

  • Manage a small caseload of up to six households experiencing literal homelessness.

  • Meet with clients document services and assist with securing permanent housing within 60 days.

  • Support employmentârelated goals and promote longâterm housing stability.

  • Attend county meetings related to RRH programming.

  • Assist with housing searches applications and landlord communication.

  • Submit monthly rent checks and coordinate rental assistance for eligible clients.

  • Provide resources and referrals to community partners for needs outside RRHs lightâtouch scope.

  • Maintain communication with landlords and partners to support client stability.

Education & Experience

  • Bachelors degree in social work sociology psychology or a related field or an equivalent combination of education and experience (subject to COA/contract requirements).

  • Minimum of two years of professional experience.

  • Strong knowledge of counseling and casework principles social service systems community resources and relevant regulations.

  • Understanding of human behavior group dynamics and social issues.

  • Ability to provide professional social work services manage time effectively build relationships and prepare detailed reports.

Certificates Licenses and Registrations

  • Valid drivers license reliable transportation and ability to meet LSS driving and insurance requirements.

  • State certification such as CSW or CPC is preferred.

Travel Requirements

  • Frequent local travel; use ofâpersonal vehicle required (mileage reimbursed).

  • Ability to travel throughout the service area up to 75% of the time; occasional overnight travel may be required.
