Expression of Interest Care Partner Lead Community Services

We are calling for an Expression of Interest for a Care Partner Lead Support at Home to join our Community Services team at Paradise. Due to parental leave there are 2 roles available:

• One position starting 20th May 2026
• One position starting 24th June 2026

In this full-time secondmentrole youll lead in team in coordinating and delivering personalised high-quality care and services that help older people achieve their goals and maintain independence at home. These roles will be concluding on Friday 15th January 2027.

Responsibilities:

• Providing Leadership Mentoring and Support to Care Partners
• Coordinating and managing individual care plans and service delivery
• Monitoring budgets and outcomes to ensure high-quality person-centred care
• Building supportive relationships with clients families and community networks
• Supporting Home Support Workers and contributing to a positive team culture
• Ensuring compliance with Support at Home program guidelines and Resthavens policies

Essential Criteria:

• Experience leading teams to meet budget KPIs whilst delivering a high quality of care
• Empathy and understanding of issues for older people their families and carers
• A commitment to supporting older people to remain at home in the community
• An ability to work with individuals to identify their needs develop personal goals and develop responsive packages of services
• Knowledge and experience of quality systems for evidence-based care
• Computer literacy
• SA Drivers Licence
• The following qualifications are highly regarded: Diploma of Nursing Certificate III in individual Support (Ageing) Certificate III Health Services Assistance Certificate IV in Aged Care Certificate IV in Disability Certificate IV in Community Services Diploma of Community Services (Case Management) Diploma of Ageing Studies and Services.
