Job Summary
Job Title:LPN / LVN
Location: Watertown NY
Duration: 13 Weeks
Schedule: Full-Time 40 hours/week (5x8)
Shift: Day Shift (8-hour shifts)
Position Overview
A well-established healthcare facility is seeking a compassionate and dependable Licensed Practical Nurse (LPN/LVN) to provide quality care in a Long Term Care (LTC) setting. The ideal candidate will support residents with chronic conditions while ensuring comfort safety and continuity of care.
Responsibilities
- Provide direct nursing care to residents in a long-term care environment
- Administer medications and treatments as prescribed
- Monitor and document patient conditions and report changes to the healthcare team
- Assist with daily living activities and support patient comfort
- Utilize electronic medical records (eMAR PointClickCare) for documentation
- Collaborate with RNs physicians and care teams to deliver patient-centered care
- Ensure compliance with facility policies and patient safety standards
Patient Population
- Dementia
- Chronic diseases
- Diabetes
Experience Requirements
- 13 years of LPN/LVN experience required
- Experience in Long Term Care (LTC) or Skilled Nursing Facility (SNF) preferred
Licensure & Certifications
- Active New York State LPN/LVN license required
- Basic Life Support (BLS) certification from the American Heart Association (AHA) required
