Job Summary
Position Overview
Inabia islooking for a Occupational Therapist to join our team in a Skilled Nursing Facility (SNF) setting. If youre passionate about patient-centered care and enjoy collaborating with a supportive team wed love to meet you.Youll help patients maximize independence with daily activities through evidence-based OT interventions and compassionate coaching.
Youll collaborate with the care team on goals discharge planning and caregiver education to support safe transitions
Key Responsibilities
- Perform evaluations develop individualized care plans and deliver therapy services based on patient needs.
- Provide daily treatments track functional progress and support discharge planning.
- Collaborate closely with therapy nursing and interdisciplinary teams.
- Complete accurate timely and compliant clinical documentation.
- Participate in skilled nursing rehab programming focused on functional recovery and safe transitions.
- Contribute to a team-based approach that prioritizes patient outcomes and quality care.
Qualifications
- Graduate of an accredited Occupational Therapy program.
- Current state OT license (or ability to obtain prior to assignment).
- Strong communication time-management and patient education skills.
- Comfort working independently while partnering with a team.
- Prior SNF experience is a plus (or a strong desire to work in post-acute care).
