Job Summary

Position Summary:

The Clinical Dietitian Homes delivers evidence-based nutrition care to residents in long-term care rehabilitation hospice and dialysis programs. Working as part of a collaborative interdisciplinary team this role conducts accurate nutrition assessments develops individualized care plans and supports ongoing monitoring to promote optimal clinical outcomes. The Clinical Dietitian partners closely with clinical staff and care teams to ensure nutrition interventions align with overall treatment goals and regulatory requirements. Through precise documentation effective communication and a team-centered approach this position supports high-quality care delivery and consistent nutrition practices across home-based and residential settings.

Key Responsibilities:
  • Conduct comprehensive nutritional assessments using clinical data laboratory values weight trends and disease-specific guidelines.
  • Develop implement and adjust individualized nutrition care plans including therapeutic diets supplementation enteral feeding and hydration strategies.
  • Monitor resident outcomes and modify interventions in response to medical changes.
  • Collaborate with interdisciplinary care teams to ensure coordinated compliant and resident-centered nutrition services.
  • Provide targeted nutrition education to residents families and staff to support disease management and diet adherence.
  • Maintain accurate timely documentation and support audits surveys and quality improvement initiatives.
  • Contribute to community outreach and consultative services through education and professional expertise.

Licenses and Certifications Required:
  • Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR) or eligible for registration as a dietitian by the CDR.
  • Licensed as a Registered Dietitian in North Dakota (ND).
  • If eligible must obtain and hold limited license in ND and within 6 months of hire date must obtain RD status by CDR credentialing.

Educational Requirements:
  • Academy of Nutrition and Dietetics registered.

Experience Requirements:
  • Prefer one year experience in health care facility.

Special Skills or Training Requirements:
  • Organize time well to meet job expectations and performance standards.
  • Teach one on one or to a large group of people.
  • Establish good rapport with other health care professionals.
  • Be adaptive to changing work situations.
  • Communicate orally with all customers.
  • Be flexible to changing work environment and to function well under pressure.

Physical Requirements:

This role requires sustained walking throughout resident care areas regular periods of sitting for documentation and the ability to lift or transport materials up to 25 lbs. The dietitian must be able to communicate clearly with residents staff and families including individuals with hearing cognitive or speech limitations. Work demands consistent attention to detail emotional resilience during challenging resident situations and the ability to multitask and prioritize effectively in a fastpaced longterm care environment. Occasional bending reaching and standing for extended periods are also necessary when assisting with assessments or educational activities.

Environmental Requirements:

The work environment includes long term care rehabilitation and hospice settings where the dietitian encounters residents with diverse and often complex medical conditions. Exposure to infectious agents cleaning chemicals bodily fluids and fluctuating noise levels is expected along with the need to follow strict infection control protocols. The role also involves interacting with residents who may have behavioral or cognitive impairments requiring patience adaptability and strong interpersonal skills. Occasional travel within the facility or to community events may be required as part of educational or consultative responsibilities.
