Community Outreach Medical Emergency Team (COMET) St Vincents Hospital provides services to people experiencing homelessness or at risk of homelessness with complex health needs. We are seeking a doctor with a broad range of medical skills to work in an outreach setting. The successful applicant will be working closely with a community nurse to assesstreatand refer patients in the community via outreach patrols and health clinics held at variousdrop incentres within the City of Sydney Local Government Area. This position would suit a doctor considering a career in general practice. The position is accredited withRACGPas a GP Extended Skills post. To provide effective medical services to patients under supervision while developingexpertisein medical practice in a complex clinical environment.

Key Accountabilities

  • The Trainee is responsible for the assessment of patients in the community and the delivery of acute medical care in a variety of community settings maintaining high levels of communication with patients regarding the nature of their condition and its treatment; communicating and co-ordinating care with the community team; arranging consultation with other staff when appropriate; support and education for interns and junior medical officers and arranging discharge and liaising with local medical officers and other community based care modalities in order to ensure the provision of optimal patient care within St Vincents Health Network.

  • Close and effective liaison with consultants will allow the doctor todemonstratetheir level of ability and understanding of patient care and enable the position holder to expand their areas of responsibility.

  • All patients managed within the hospitals of the health service are under the direct care of a specialist medical practitioner. All decisionsregardingtheir care are to be clearly communicated with the specialist responsible for that care.

  • Provide high quality healthcare to people experiencing or at risk of homelessness through the provision of outreach health clinics andstreet basedcare.

  • Provide evidence-based healthcare brief intervention assessment intervention care planningadvocacyand referralin order toassistclients to access mainstream and specialist healthcare to improve their health and wellbeing outcomes.

  • Proactivelyestablishand maintain partnerships with clients local and specialist healthcare providers in the community to build and strengthen client health support networks

  • Working strongly within the multidisciplinary team to provide culturally safeappropriate inclusive and wholistic health care.

  • Operate according to the principles of harm minimisation trauma-informedcareand the strengths model.

  • Participate in multidisciplinary Clinical Review Meetings and coordinate care planning to ensure delivery of safeeffectiveand collaborative health care.

  • Engage in regular professional development including participation in clinical supervision

  • Participate in service development and quality improvement activities.

  • Provide education to MedicalStaff within the hospital with a focus on capacity building and enhancing service delivery for people experiencing homelessness.

  • Ensure that all documentation servicereportingand evaluation requirements are met effectively andin a timely manner.

Essential Requirements

  • MBBS or equivalent currentlyregisteredor eligible for registration with the Medical Board of Australia

  • Completion of at least three (3) postgraduate years of medical practice.

  • Valid NSW Working with Children Check.

  • Cleared National Police Check.

  • Demonstrated commitment and appointment to relevant GP training program. This is most likely to be through theSIGPET(Sydney Institute of General Practice Education and Training).

  • Ensure compliance across all relevant standards of accreditation and legislative requirements within areas of responsibility or as delegated by the CEO St Vincents Health Network Sydney.

  • Ensure facilitiesoperateat all times in compliance with the Catholic Health Australia Code of Ethical Standards for Catholic Health and Aged Care Services in Australia and relevant legislation.

  • Ensure compliance with relevant legislationstandardsand industrial instruments.

  • Operate within the delegated responsibilities and authorities as set by St Vincents Health Australia.

  • Ensure relevant personal qualification registrations and memberships aremaintainedat the required level.

  • Ensure that employees are compliant with mandatory training requirements.

Selection Criteria

MBBS or equivalent currentlyregisteredor eligible for registration with the Medical Board of Australia.

Completion of at least three postgraduate years and working towards completion of relevant College examinations anddemonstratedcommitment and appointment to relevant GP training program.

Demonstrated understanding of the health impacts of homelessness and the ability to provide culturally safeholisticand inclusive healthcare to priority populations.

Demonstrated effective communication skills and the ability to work within the multidisciplinary team and with a range of external and internal stakeholders.

Demonstrated commitment to clinical supervision ongoing professional development anddemonstratedeffective communication and interpersonal skills.

Personal integrity anddemonstratedcommitment to the Philosophy Mission and Values of MaryAikenheadMinistries and St Vincents Health Australia.

Demonstrated effective computer literacy with email and an electronic patient record system.

Current NSW Drivers License.
