The Patient Scheduler & Transportation Dispatcher is a non-clinical office-based role responsible for scheduling medical appointments and coordinating transportation for participants. This position involves creating and maintaining schedules dispatching drivers managing transportation requests and ensuring efficient routing. The role also includes administrative tasks such as ordering supplies sending appointment notices and managing medical records requests.

Responsibilities:
- Creates and maintains clinic schedules for PCPs RNs and LPNs including booking exam rooms and nursing care areas.
- Maintains clinic calendar and inputs return visit requests and semi-annual assessments.
- Schedules appointments for participants with internal and contracted medical specialists.
- Coordinates medical appointment schedules with ADH and other disciplines.
- Maintains contact with specialist providers regarding schedules.
- Dispatches drivers to transport participants to and from medical appointments and adult day health visits.
- Manages all transportation requests and coordinates efficient routing to maximize transportation resources.
- Creates scheduled routes for transportation and makes adjustments to meet unanticipated changes.
- Works with the Transportation Manager and drivers to determine the safest routes.
- Utilizes transportation software and medical records software to track and schedule medical appointments.
- Obtains outside lab work medical imaging and external vendors as needed.
- Ensures necessary paperwork is completed and submits transportation requests.
- Orders clinic supplies maintains a system of packing slips and invoices and coordinates delivery of supplies.
- Sends out appointment notices and makes reminder calls to participants/caregivers/escorts.
- Requests records from hospitals nursing homes providers and external specialists.
- Creates packets for ER hospital Nursing home and AFH admissions.
- Covers breaks and lunch for the receptionist as needed.
- Sorts and distributes mail and physician faxes.
- Provides back-up support for the receptionist at all sites.
- Operates office systems and equipment including computers telephones copiers and fax machines.
- Coordinates work with internal and external contacts as required.

Required Skills & Certifications:
- Associates degree preferred.
- 2 years of experience in scheduling medical appointments and/or dispatching medical transportation.
- Proficiency in Outlook.
- Knowledge of Microsoft Office products.
- Excellent organizational and interpersonal skills.
- Strong verbal and written communication skills.
- Ability to communicate effectively in English.
- Associates degree preferred.
