Job Summary
Navitas Healthcare LLC is seeking a dedicated Licensed Master Social Worker (LMSW) to provide comprehensive social work services in an acute care setting. The ideal candidate will support patients and families through assessment care coordination discharge planning and psychosocial interventions in a fast-paced inpatient environment.
Key Responsibilities
- Conduct psychosocial assessments and develop patient-centered care plans
- Provide counseling and emotional support to patients and families
- Coordinate discharge planning and connect patients with community resources
- Collaborate with interdisciplinary healthcare teams to ensure continuity of care
- Maintain accurate and timely documentation in compliance with regulations
Required Qualifications
- Masters Degree in Social Work (MSW)
- Active LMSW license issued by NYSED (Primary Source Verified)
- Minimum 1 year of LMSW experience
- Experience in inpatient medicine surgery or med/surg settings
- Acute care experience required
Additional Information
- Full-time schedule: 5 days/week 7 hours/day
