Job Title: Director of Case Management
Location: North Richland Hills TX 76180
Schedule: Full-Time Days (Weekdays Only No Weekends)
Shift: 710 Hour Shifts
Employment Type: Permanent / Direct Hire
Reports To: Senior Leadership Team
Position Summary:
We are seeking an experienced Director of Case Management to lead and oversee all case management operations within an acute care hospital setting. This role is responsible for ensuring effective care coordination discharge planning regulatory compliance and improved patient outcomes. The Director will collaborate closely with executive and clinical leadership to drive performance and foster a strong team-oriented culture.
Facility Overview:
- Acute care hospital with Level III Trauma designation
- Approximately 164 licensed beds with planned rehabilitation expansion
- Annual patient volume of 32000
- Team structure: 10 FTEs PRN staff
Key Responsibilities:
- Lead and evaluate case management operations including staffing models onboarding and competencies
- Ensure efficient care coordination and discharge planning processes
- Monitor departmental performance and drive process improvement initiatives
- Collaborate with finance and leadership teams to meet operational and financial goals
- Ensure compliance with policies SOPs and regulatory standards
- Allocate resources to meet productivity and quality benchmarks
- Build strong partnerships with clinical administrative and support teams
- Promote adherence to organizational mission values and code of conduct
- Participate in on-call rotations and extended hours as needed
Required Qualifications:
- Bachelors Degree (required)
- Masters Degree in Nursing Healthcare Administration or Business Administration (preferred)
- Active licensure: RN LCSW or LMSW (required)
- Minimum 3 years of acute care hospital case management experience
- Minimum 2 years of leadership experience in case management
