Job Summary
Evaluates and assesses patients to establish functionalmeasurable goals to treatmentaccordingto a written plan of care to meet established goals.Plans for dischargeand communicatesfindings and progress accordingto professional licensure/certificationscope of practiceand departmental/disciplinepolicies and procedures. Produces other reports as needed. May supervise clinical students and support staff including certified/licensedassistants techniciansand to the ASHA Code of Ethics. Provides a caring relationshipthat facilitateshealth and healing. Performs other duties as assigned.
Job Responsibilities Evaluates and assesses the patient based on subjectiveand objectiveinformation. Communicatesresults of the evaluation to patient and other caregivers. Provides treatment accordingto establishedgoals on plan of care. Documents interventions/carein the patientsmedical record. Evaluates effectiveness of plan of care and reassesses patientsfunctionalstatus as needed. Provides discharge planning informationwhich includes patient/familyeducationfollow-up andreferral to communityresources. Contributesto the developmentof staff and students. Participates in activitiesdesigned to improve health care delivery. Completes assigned goals. |
Experience
Minimum required - Entry Level
Minimum Required - Education - Completion or an accredited Masters or Doctorate degree in Speech language Pathology Communicative Disorders or Speech and Hearing Science.
Special Skills
Minimum Required - Basic computerliteracy skillssuch as keyboardingusing amouse and navigating sites.
Licensure Description
SuccessfulcompletionofNational Examinationin SpeechLanguage a clinical fellow with stateboard of communciation disorders and sciences.
