Age Specific Responsibility

Must be able todemonstratethe knowledge and skills necessary to provide careappropriate tothe age of the patients served in assigned department. Assumes full responsibility for evaluatingpatientsneedfor physical therapy.Plans and executes individual treatment programsin accordance withthe State of Michigan licensure and PT scope of practice.Keeps treatment skills upgraded through continuing education evidence-based research and program development.Perform job responsibilitiesin accordance withthe programs strategicobjectivesand goals to carry out organizational strategicobjectives.

  • Staff will provide patient care treatment and services within the scope of their license certification or registration and as required by law and regulation.

  • Evaluates patient condition.

  • Establishes treatment goals in collaboration withpatientand family asappropriate and in collaboration with the treatment team. Treatment goals should be interdisciplinary and patient-centered whenever possible. Establisheswrittentreatmentplan andadministers treatment for each patient according to the stated goals.Delegates responsibilities tophysical therapist assistants and rehabilitation technicians asappropriate.

  • Re-evaluatespatientsstatus and effects of treatment.Revisestreatmentplanas necessary. Reviews revised goals withpatientand family asappropriate.

  • Evaluatespatientsneed for medical equipment and orders theappropriate equipmentas necessary.Evaluates equipment upon delivery and instructspatientpatientsfamilyand health care workers on its use.

  • Participates in planning for patient discharge by considering and communicating with patient family community support/servicesand environmental limitations.

  • Demonstrates competence in relevant software and hardwareto meet job responsibilities.(Q)

  • Documents patients status and progress throughinitialevaluation progress notes and discharge summary according toestablishedinternal/external time standards and adherence to reimbursement guidelines.(Q)

  • Communicates with others to integrate physical therapy treatment with all aspects of patient care.(QS)

  • Participates in conferences/discussions as needed and exchange of written records with other interdisciplinary team members to integrate physical therapy treatmentplan.

  • Instructs the patient and the family in physical therapy procedures as partof providingpatient and familyeducationincluding writteninstructionsas necessary.

  • Attends various meetings and in-services as designated.(QP)

  • Follows all hospital policy and procedures including but not limited to patient confidentiality and infection control.(Q)

  • Meets or exceeds program productivity expectations.(F)

  • Provides discipline specific coverage (following cross training) within the Mary Free Bed system asrequested.(FS)

  • Adheres to the Mary Free Bed Standards of Behavior when working with patientscoworkersand members of the community.(QSP)

  • Supports program operations as assigned.(Q S P G F)

  • May perform the following items to meetprogramor hospital needs.(Q S P G F)

  • Participates in orientation and training ofnew staff.(Q P)

  • Serves asclinicalinstructor for physical therapy students.Supervises teaches and evaluatesskillsof students. Communicates with participating universitiesregardingskill levels of students.(S)

Essential Job Qualifications

  • Graduate of Commission on Accreditation in Physical Therapy Education (CAPTE)

  • Licensure isin every US State andTerritorythe Therapist is practicing. Licensure requirements should be met according toindividual Statelaws.

  • Possesses and maintains BLS certification to meet department/program protocol.
