Job Summary

Intake Coordinator

The first friendly voice of care.

Our Intake Coordinator is the welcoming front door to Health at Home. This role is often the very first interaction a patient family member or referral partner has with our organization and that makes it incredibly important.

You are the calm in the chaos the organized one behind the scenes and the friendly voice that helps people feel supported heard and confident in their next steps. You connect referrals to care questions to answers and families to the services they need all while keeping things accurate timely and compliant.

What Youll Be Doing

Intake & Referral Management

You will receive and process incoming Home Health and Private Duty referrals from hospitals physician offices families and community partners.
You will answer private duty inquiry calls gather initial information and route leads to the right team to support timely care and conversion.
You will review referral documentation for accuracy and completeness and follow up when information is missing.
You will communicate referral status next steps and service availability clearly to patients families referral partners and internal teams.

Insurance & Benefits Verification

You will verify insurance eligibility and benefits according to payer and agency guidelines.
You will document coverage details limitations and authorization requirements accurately.
You will explain benefit information in a clear respectful and easy-to-understand way.

Coordination & Internal Collaboration

You will work closely with clinical scheduling operations and sales teams to move referrals smoothly into active services.
You will coordinate with leadership and clinicians on referral appropriateness timing and care needs.
You will serve as a bridge between referral sources and internal teams to ensure responsiveness and service excellence.

Communication & Customer Experience

You will provide timely compassionate and professional communication to patients families and partners.
You will maintain strong relationships with hospitals physician offices discharge planners and community partners.
You will represent Health at Homes values professionalism and brand in every interaction.

Documentation & Systems

You will enter and maintain referral intake and benefit information in our electronic systems.
You will keep organized referral records and communication logs.
You will monitor referral pipelines and follow-ups to support timely service starts.

Compliance & Confidentiality

You will protect patient privacy and comply with HIPAA and company policies.
You will follow all federal state and agency regulations for home health and private duty services.
You will adhere to all standards within the organizations Compliance Program.

What Were Looking For

High school diploma or equivalent required. Healthcare-related education is a plus.
Experience in healthcare intake referrals admissions or insurance verification is preferred.
Familiarity with home health and or private duty workflows is strongly preferred.
Comfort with insurance verification processes and medical terminology.
Strong organization and time-management skills with the ability to juggle multiple referrals.
Excellent communication skills with a warm professional phone presence.
Ability to work independently use good judgment and collaborate in a fast-paced environment.
