Qualifications :
- Licensed Physical Therapist in the state of New York or eligible.
- Ability to evaluate a patients need for physical therapy and develop an appropriate treatment plan.
- Provide appropriate care and treatment to patients based on physician care plan agency policy and professional standards of care.
- Experience instructing family members PTAs Home Health Aides and other caregivers in appropriate physical therapy techniques.
- Mandatory Certifications: First Aid AED & CPR Certifications.

The anticipated base hourly range for this position is $40-60. Salary is based on various factors including relevant experience knowledge skills other job-related qualifications and geography. Additionally this position is eligible for incentive compensation. The Companys incentive compensation plan is subject to change.
