Position Urgency: Quick Start
Schedule: Full-time 36 hours/week (Monday Friday with alternating weekend rotation)
Setting: Skilled Nursing Facility (SNF)
We are seeking an experienced Occupational Therapist to join a skilled nursing rehabilitation team. The ideal candidate will provide high-quality therapy services to residents requiring restorative post-acute and long-term care rehabilitation. This is a contract assignment offering consistent hours in a supportive clinical environment.
Key Responsibilities- Deliver evaluation treatment and documentation for patients in a Skilled Nursing Facility.
- Develop individualized treatment plans to improve functional independence and quality of life.
- Collaborate closely with Physical Therapists Nursing staff and interdisciplinary team members.
- Maintain accurate and timely documentation within the facilitys EMR system.
- Participate in care planning progress reviews and family education as needed.
- Provide therapy services in alignment with federal state and facility guidelines.
- 36 hours per week
- Monday Friday primary schedule
- Alternating weekend rotation required
- Active California Occupational Therapy license (must be current).
- Minimum 1 year of experience preferably in a Skilled Nursing Facility setting.
- Familiarity with SNF rehabilitation programs and geriatric patient care.
- Experience using EMRs; PointClickCare experience preferred.
- Strong communication teamwork and documentation skills.
- Ability to work independently and effectively manage a caseload.
- Skilled Nursing Facility experience highly preferred.
- Experience collaborating with multi-disciplinary rehab teams.
- Contract length: 6 weeks
