Navitas Healthcare LLC is seeking a compassionate and reliable Travel Licensed Practical Nurse (LPN) to join a Skilled Nursing Facility (SNF) in Hancock MI. In this role you will provide high-quality patient-centered care while supporting residents comfort safety and overall well-being. This opportunity is ideal for LPNs who excel in fast-paced environments and are committed to delivering exceptional care.
Key Responsibilities
- Deliver direct patient care in accordance with individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patient conditions and report any changes to the RN or physician promptly
- Assist residents with activities of daily living (ADLs) including hygiene mobility and feeding
- Maintain accurate and timely patient records in compliance with facility standards
- Adhere to infection control protocols and safety guidelines
- Collaborate with interdisciplinary healthcare teams to optimize patient outcomes
Required Qualifications
- Active LPN license (state-specific or compact)
- 12 years of experience in a Skilled Nursing Facility (SNF) or similar setting
- Strong clinical skills and a patient-focused approach
- Ability to adapt quickly in a travel assignment environment
Education
- Diploma or Certificate in Practical Nursing from an accredited program
Licensure / Certification:
- AHA BLS Certification
- COVID-19 Vaccination Card
- Driving License
Preferred Experience
- Experience in long-term care rehabilitation or SNF settings
- Familiarity with Electronic Medical Records (EMR) systems
